MAIN FUNCTION:
The Nurse Care Coordinator will be a member of the CDPHO Management Team and will attend weekly management meetings, as needed, in order to participate in the development and execution of various CDPHO strategic initiatives.
Optimize the delivery of care for a limited group of patients at high risk of poor health outcomes and high health care costs. The NCC will work closely with the patient's PCPs, and will develop a plan of care in concert with the PCP and appropriate Specialists. This will involve assessing the broad range of the patient's needs: medical, psychological, financial, and educational; and designing a plan of care to meet those needs, utilizing the full range of resources available. Initially, 100% of the incumbent's time will be work to establishing the care management program by researching intake and care coordination protocols as well as feedback mechanism to the primary care physicians and documentation requirements. As health plan members are identified as needing care coordination, the incumbent's time for management of the program will change to 25% management time and 75% health plan member coordination time.
MINIMUM REQUIREMENTS:
" RN and BSN or in process;
" At least 2 years previous management experience which a proven track record of program development and implementation.
" 3+ years experience in one or more of: Case Management, in-hospital or other; or Chronic Disease Management Program; Visiting Nurse agency; Hospice; or similar.
" Additional training and experience with Clinical Pathways and Guidelines, and Quality Improvement.
" Ability to use MIDAS and similar software programs.
" Excellent interpersonal, communication and organizational skills.
" Knowledge of the variety of health care agencies and resources available in Hampshire County a plus.
